29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Employees engaged in roofing activities at a residential building under construction at 22318 W. 58th Street, Shawnee Kansas were exposed to struck-by and fall hazards in that employees were not instructed in the recognition of construction hazards and the methods to mitigate those hazards, to understand when to use fall protection, eye protection, and head protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 22318 W. 58th Street, Shawnee Kansas, employees engaged in residential re-roofing activities were exposed to fall hazards of approximately 20 feet 2 inches from the roof to the ground below while accessing and exiting the roof in that the ladders did not extend at least 3 feet above the upper landing and were not secured to the residence/structure.
