29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  At the jobsite located at 22506 W. 61st Street, Shawnee Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 14 feet 8 inches from the roof to the ground below in that fall protection was not utilized.  The Alexandro's Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11) which was contained in OSHA inspection number 1487888, citation number 1, item number 1 and was affirmed as a final order on 03-01-2021, with respect to a workplace located at 4101 S. MO Hwy 7, Blue Springs, Missouri.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at its top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:   At the jobsite located at 22506 W. 61st Street, Shawnee Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 14 feet 8 inches from the roof to the ground below while accessing and exiting the roof in that the ladders were not secured to the residence/structure.  The Alexandro's Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1) which was contained in OSHA inspection number 1487888, citation number 1, item number 2 and was affirmed as a final order on 03-01-2021, with respect to a workplace located at 4101 S. MO Hwy 7, Blue Springs, Missouri.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with the standards set forth in 29 CFR 1926 as evidenced by the violation(s) noted during the course of the inspection:  At the jobsite located at 22506 W. 61st Street, Shawnee Kansas, employees engaged in residential roofing activities with the use of air compressor, pneumatic nail guns, ladders were exposed to struck by and fall hazards in that the employer had not developed nor implemented safety and health programs that provide construction hazard recognition guidelines to mitigate those hazards.
