(a) On or about July 17, 2024, at 3827 S Bluestem Dr, Oak Creek, WI 53154, employees conducting roofing activities on a residential roof with a 10:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of 20 feet 2 inches on the West side of the residential home.

Juan Carlos Alvarez Govea dba Juan Carlos Alvarez Govea was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1715890, citation number 1, item number 2 and was affirmed as a final order on April 10, 2024, with respect to a workplace located at 3800 Sunnycrest Dr. Brookfield, WI 53005.
