29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about July 18, 2024, at the jobsite located at 725 E Elm Rd, Oak Creek, WI 53154, employees were installing underlayment on the upper roof without the use of fall protection. Employees were exposed to a fall hazard of approximately nine (9) feet to 15 feet from the roof to the ground below.

29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces: (a) On or about July 18, 2024, at a jobsite located at 725 Elm Rd, Oak Creek, WI 53154 the employer did not ensure the portable ladder used by employees to access the upper roof from the ridge of the garage was equipped with slip resistant feet, exposing employees to a fall hazard of approximately six feet to the lower roof.
