29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  (a) On or about July 17, 2024, the employer did not ensure frequent and regular inspections of the job site were conducted by a competent person to ensure hazardous conditions were found and corrected. Hazards identified included but were not limited to: fall protection not being worn, fall protection systems were not properly installed,  ladders were not installed and/or used properly.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about July 24, 2024, employees were installing underlayment on the upper roof without the use of fall protection. Employees were exposed to a fall hazard of approximately 18.5 feet to the ground below.   (b) On or about July 24, 2024, employees were performing tear-off of existing shingles and underlayment on the lower roof without the use of fall protection. Employees were exposed to a fall hazard of approximately nine (9) feet to the ground below.

(b) On or about July 24, 2024, the employer did not ensure the portable extension ladder used by employees to access the lower roof of the residential home extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately nine (9) feet to the ground below.

29 CFR  1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  (a) On or about July 24, 2024, the employer did not ensure the separated upper section of a portable extension ladder used by employees to access the upper roof from the ridge of the garage was equipped with slip resistant feet and that the ladder extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately nine (9) feet to the lower roof.
