29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  15 Judd Ave., South Hadley, MA: On or about July 22, 2024, the employer did not ensure employees were protected from fall hazards of 8 feet 5 inches during a residential construction project that involved shingle replacement.  WR General Construction, Inc was previously cited for a violation of this occupational safety and health standard, 1926.501(b)(13), which was contained in OSHA inspection number 1637555, citation number 1, item 2a, and was affirmed as a final order on February 24, 2023, with respect to a workplace located at 142 Bliss Rd., Longmeadow, MA.

29 CFR  1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:  15 Judd Ave., South Hadley, MA: On or about July 22, 2024, the employer had not ensured that ladders being used to access a residential roof were used only for the purposes they were designed.  A material hoist (shingle lift) ladder, not designed for climbing, was used by an employee to descend from a rooftop while performing shingle replacement activities.  WR General Construction, Inc was previously cited for a violation of this occupational safety and health standard, 1926.1053(b)(4), which was contained in OSHA inspection number 1637555, citation number 1, item 4, and was affirmed as a final order on February 24, 2023, with respect to a workplace located at 142 Bliss Rd., Longmeadow, MA.
