29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: A) JR Cruz Corp. - 89 Maguire Ave., Staten Island NY 10309 - On July 22, 2024, an employee was using a Stihl model TS 420 gas powered cutoff/demo saw to cut a ductile iron pipe in the excavation when the sawblade, jammed and kicked back and the employee sustained a facial injury from the incident. The employer did not ensure each employee was trained on the reactive forces and kickback while operating the Stihl gas powered cutoff/demo saw and how to mitigate or eliminate the hazard. Condition occurred on July 22, 2024

29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. A) JR Cruz Corp. - 89 Maguire Ave., Staten Island NY 10309 - On July 22, 2024, an employee operating a Stihl demo saw to cut 12" ductile iron storm water pipe in place was not wearing face protection, during cutting operations, the saw kicked back and the employee suffered laceration to the right side of his face and jaw. Incident occurred on July 22, 2024
