29 CFR  1926.1203(e)(2)(ii):When entrance covers were removed the opening was not immediately guarded by a railing, temporary cover, or other temporary barrier that would prevent an accidental fall through the opening and that would protect each employee working in the space from foreign objects entering the space.  A) JR Cruz Corp. - NW Corner Cedar Grove Ave. & New Dorp Lane., Staten Island NY 10306 -   A steel grating was removed to gain access into a catch basin that was approx. 7 feet deep. Th opening was not protected and allowed the grating to fall striking the employee who was inside performing cleaning activities with a vac truck hose. The grating fractured the employee's toes.   Incident occurred on January 07, 2025
