29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress. a) Jobsite located at or near 23 W Girard in Philadelphia, Pennsylvania- On or about July 25, 2024, the employer allowed employees to use cross braces on tubular welded frame scaffolds as means of access. An employee climbed on the cross braces of the scaffold to attach a pulley to the scaffold, exposing the employee to fall hazards of approximately 19 feet.

29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Jobsite located at or near 23 W Girard in Philadelphia, Pennsylvania- On or about July 25, 2024, the employer did not ensure that employees conducting masonry activities from a tubular frame scaffold were provided fall protection systems or guardrail systems. The employer allowed employees to work on a scaffold with missing guard rails and no fall protection, thereby exposing employees to fall hazards of approximately 13 feet.
