29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly Bldg 505: On and/or before July 25, 2024, the employer did not ensure that the portable fire extinguishers throughout their warehouse were inspected at least monthly. This condition potentially exposed employee to fire hazard(s). California Army National Guard was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(2), which was contained in OSHA inspection number 1639497, citation number 3, item number 1, issued on January 13, 2023, which was affirmed as a final order on January 31, 2023 with respect to a workplace located at 950 N. Cucamonga Ave. Ontario, CA 91764.

The bases of the industrial rack system(s) were not anchored with anchor bolts to the concrete floor. This condition exposed employee(s) to rack system(s) potentially tipping over creating crushing hazard(s).

Administrative office: On July 25, 2024 and times after, the employer failed to provide copies of the requested injury and illness records for the calendar years 2021, 2022, 2023 when requested both verbally and in writing.
