29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color: a. Administration Building - On or about July 30, 2024, the exit signs throughout the administration building were metal signs that did not have emergency lighting and were not self-illuminating or designed to be photoluminescent. b. Maintenance Shop - On or about November 8, 2024, the exit sign leading from the maintenance shop to the outside of the building was no self-illuminating or provided with alternative emergency illumination. c. Mill exit to maintenance shop - On or about November 8, 2024, the exit sign leading from the mill into the maintenance shop for discharge from the maintenance shop to the exterior was not illuminated or provided with alternative emergency lighting. 29 CFR 1910.119(c)(2): The employer did not consult with employees and their representatives on the conduct and development of process hazards analyses (PHA) and on the development of the other elements of process safety management in this standard. a. Mill Blending Area - On or about July 30, 2024, the October 2022 PHA revalidation did not include consultation or participation by hourly employees as written in the employers written program for how they would involve employees in the process. Failure to include employees that operate and maintain the system can lead to oversite of potential hazards that may lead to the release or exposure of employees to Chlorine. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Mill Blending Area - On or about July 30, 2024, the piping and instrumentation diagram 4140-PD-01 did not include a 3" piping circuit 3"-PA-0540-03-010-00CS00-N/A, off of PSV 01 between 4140-BL-001 blower and the collection hopper Airlock. Failure to maintain accurate P&ID's can result in improperly sizing of replacement parts, slowed response and potential exposure to chlorine liquid or gas. b. Mill Blending Area - On or about July 30, 2024, the piping and instrumentation diagram 4140-PD-01 did not include the piping sizes and information on the piping circuit going to the general suction off of 4"-P-4140-01-003-AA-N/A, 4"-P-4140-01-002-CS-N/A, and 4"-P-4140-01-004-AA-N/A, the P&ID reads ?-P-?-?-?-?-?. Failure to maintain accurate P&ID's can result in improperly sizing of replacement parts, slowed response and potential exposure to chlorine liquid or gas. Corrected During Inspection
