29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) E-Coat & Liquid Line; Stages 1 through 14: On December 16, 2024, and at times prior, the employer exposed employees to fall hazards of approximately 8 feet, in that the employer did not ensure that employees had fall protection while working on platforms within the coating machine to retrieve parts that had fallen into the immersion tanks.

29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a) Shipping & Receiving Department: On August 13, 2024, and at times prior, the employer exposed employees to struck-by hazards, in that a Hyster H50FT sit-down forklift (Serial No. L177V13375L) with damaged brakes was not removed from service as employees operated the equipment to store and retrieve products and materials.   b) Shipping & Receiving Department: On August 13, 2024, and at times prior, the employer exposed employees to struck-by hazards, in that a Taylor GT60L sit-down forklift (Serial No. XGT466L-0015-10030) with a damaged horn and damaged propane tank strap was not removed from service as employees operated the equipment to store and retrieve products and materials.

29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.   a) Production Department; Load and Unload Areas: On August 13, 2024, and at times prior, the employer exposed employees to trip hazards, in that the employer did not ensure that the work floor was clear of hooks as employees used them to hand metal parts from an overhead conveyor system.  b) Production Department; Aisle Between Load Area and Lab: On August 13, 2024, and at times prior, the employer exposed employees to trip hazards, in that the employer did not ensure that the work floor was clear of uncovered electrical cables as employees traversed the area to access stages 5 through 10 of the coating machine.
