29 CFR  1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations:  a) Inside Tank System Stages 1-14; On or about August 13, 2024, and at times prior, Maintenance Technicians and Production Associates were exposed to engulfment and mechanical hazards, in that the employer directed employees to perform work inside tank system stages 1-14 and did not implement a written permit confined space entry program that specified acceptable entry conditions; provided each authorized entrant or that employee's authorized representative with the opportunity to observe any monitoring or testing of permit spaces; verified that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry; provide procedures to summon rescue services in the event that authorized entrants require emergency assistance.
