29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about July 31, 2024, employees using the Azimuth electric press (Model #48P and Serial #2597) were exposed to crushing and amputation hazards, in-that the employer did not ensure the point of operation was properly guarded. A guard was installed at the front point of operation, but did not prevent employees from accessing the sides of the point of operation.
