OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that it allowed liquid ammonia to remain in a pressure vessel that was substantially rusted, had 'old' pressure relief valves, and may have been over-pressurized. Location: Outdoor ammonia storage vessel located outside the Vibe Room; Pressure relief vent discharge pipe for the annealing furnace. Instance A: Pressure relief vessels ( PRV ) on the ammonia tank were not maintained in accordance with CGA G 2.1 0 2014. Instance B: An ammonia tank PRV lacked a rain cap or similar device. Instance C: The annealing furnace pressure relief discharge vent terminated at below the height of roof.
