29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) (a) On and before July 31, 2024, and at times prior, R4 Roofing, as the controlling employer, did not ensure a safety program was developed and implemented for the work environment, and that it included regular inspections made by a competent person of the jobsite, materials, and equipment. Subcontractor employees were working next to the edge of the roof on a one-story residential home without the use of a fall protection system and had a portable ladder set up incorrectly. The subcontractor employees were removing roofing material on the sloped roof. This condition exposed employees to a fall hazard greater than 10 feet to the ground below.
