(a) On the vertical steel beam near Beam Warper #8 : On or about September 5 , 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that the electric cord extending from the junction box to the floor had damaged insulation thereby exposing employees while walking through the Warping area.
