29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system: (a) On July 30, 2024, at 1025 14th Ave South, Fargo, ND, employees installing sheathing on a trussed roof were exposed to a fall while working at heights greater than 6 feet without the use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: (a) On July 30, 2024, at 1025 14th Avenue South, Fargo, ND employees were exposed to hazards associated with potential displacement or tipping of an extension ladder used to ascend or descent from an elevated walking-working surface to install sheathing on a roof, the ladder did not have rails that extended three feet above the upper landing surface or secured to the roof.
