29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) 6955 Chrisphalt Dr., Bath, PA 18014 - facility wide - On or about August 6, 2024 - tires on the 2012 Yale, Serial # A910V19228J Powered Industrial Truck (PIT) were found to be worn with areas of cracking and chunking. These conditions can cause unsafe travel and loss of control of the PIT resulting in an employee injury. b) 6955 Chrisphalt Dr., Bath, PA 18014 - facility wide - On or about August 6, 2024 - tires on the Hyster 50VX, Serial # A910V10971K Powered Industrial Truck (PIT) were found to be worn with areas of cracking and chunking. These conditions can cause unsafe travel and loss of control of the PIT resulting in an employee injury.

29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section: a) 6955 Chrisphalt Dr., Bath, PA 18014 - On or about August 6, 2024 - Back up lights were removed from forklift, 2012 Yale, Serial # A910V19228J creating the inability for others to see the forklift while driving backward. b) 6955 Chrisphalt Dr., Bath, PA 18014 - On or about August 6, 2024 - Back up lights were removed from forklift, Hyster 50VX, Serial # A910V10971K creating the inability for others to see the forklift while driving backward.
