This was most recently documented on or about August 8, 2024 at 2535 Edward Babe Gomez Ave, Omaha, Nebraska. SCBAs model MSA Airhawk II used by the on-site HAZMAT response team were not available during a formaldehyde spill on August 7, 2024. The employer had sent multiple MSA Airhawk II, which the employer expected employees to use in the case of a HAZMAT situation, to be certified/inspected off-site. Employees were left with 0 MSA Airhawk II on-site to respond to the formaldehyde spill.
