29 CFR 1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: On or about August 1, 2024, the employer did not ensure the developed and documented hazardous energy control procedure for the 60" slitter machine line clearly and specifically outlined the scope, purpose, rules, and techniques to be utilized while employees performed service and maintenance activities, such as but not limited to repairing the hydraulic cylinder of the slitter line peeler table machine. The energy control procedure did not include a specific statement that the intended use of the procedure was for controlling the hazardous energy sources of the individual machines and equipment in each zone of the 60" slitter line or the hazardous energy sources of the entire 60" slitter line when conducting service and maintenance activities [per 1910.147(c)(4)(ii)(A)]. The energy control procedure did not clearly identify the specific procedural steps for blocking and securing the peeler table's hydraulic cylinders or the peeler table's gravitational energy source or specific procedural steps for shutting down, isolating, blocking, and securing individual machines and equipment in each of the individual zones of the 60" slitter line when performing service and maintenance activities [per 1910.147(c)(4)(ii)(B)]. The energy control procedure did not clearly identify the specific procedural steps for the placement and removal of lockout devices for the hydraulic cylinders of the peeler table or specific procedural steps for the placement, removal, or transfer of lockout devices when performing service and maintenance of the individual machines and equipment in each of the individual zones of the 60" slitter line [per 1910,147(c)(4)(ii)(C)].
