29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Rail Mill, North stamp wheel:  On or about August 9, 2024, and at times prior, the employer did not ensure that employees engaged in maintenance activities utilized energy control procedures to isolate and de-energize the sled while the face plate was removed to replace a retaining pin on the north stamp wheel. This condition exposed employees to caught-between and crushed-by hazards resulting in a fatality.

29 CFR  1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:  (a) Rail Mill, North Stamp Wheel: On or about August 9, 2024, and at times prior, the employer did not ensure the sled and north stamp wheel was shut down for maintenance procedures prior to removing the face plate to replace a retaining pin. This condition exposed employees to caught between and crushed by hazards resulting in a fatality.

29 CFR  1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished:  (a) Rail Mill, North stamp wheel: On or about August 9, 2024 , and at times prior, employees replacing a retaining pin on the North stamp wheel did not verify that the sled had been isolated and deenergized. This condition exposed employees to caught-between and crushed-by hazards resulting in a fatality.

29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a) Rail Mill, North stamp wheel: On or about August 9, 2024, and at times prior, the employer did not ensure that each authorized employee locked out before working on the north stamp wheel. Employees replacing a retaining pin on the stamp wheel did not affix their personal locks to the group lock box prior to working on the wheel.  This condition exposed employees to caught-between and crushed-by hazards resulting in a fatality.

29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:  (a) Rail Mill, North stamp wheel: On or about August 9, 2024, and at times prior, the employer did not ensure that energy control procedures included the specific procedural steps for the placement and removal of lockout devices, and specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices. This condition exposed employees to caught-between and crushed-by hazards.

29 CFR  1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  (a) Rail mill electrical room: On or before August 9, 2024, and at times prior the employer did not ensure that maintenance electrical technicians wore appropriate electrical protective equipment when locking out energized 480-volt electrical circuits. Employees opened and closed a knife switch on the stamp wheel electrical panel (CP-6C) in close proximity to energized conductors without personal protective equipment such as an arc-rated face shield, arc-rated balaclava and voltage rated gloves. This condition exposed the employees to hazards such as shock, electrocution, arc blast, and arc flash.
