(a) On or about August 12, 2024, at 10402 S Rosemont Lane, Oak Creek, WI 53154, subcontractor employees installing a roof on a residential home with an approximately 10:12 pitch roof were not using fall protection. This exposed employees to a fall hazard of approximately 9 feet 8 inches from the roof eve to the ground below on the back (East) side of the home. The controlling employer, Cornerstone Roofing Inc, did not ensure that the subcontractor employees were wearing fall protection.
