This hazardous condition exposed employees to severe injury due to the potential of fire and vapor cloud explosions hazards posed by a DMA release.  Abatement Certification Required.

This hazardous condition exposed operators to severe injury due to the potential of fire and toxic chemical overexposure hazards posed by a DMA release.  b) Formaldehyde Storage Tank - On or about August 13, 2024, the procedure formaldehyde Unloading (TAGOP-002) did not discuss the specific steps operators were required to take to correct or avoid a deviation from the upper and lower operating limits for fill level and temperature of the formaldehyde storage tank. This hazardous condition exposed operators to severe injury due to the potential of fire and toxic chemical overexposure hazards posed by a formaldehyde release.  Abatement Certification Required.
