The employer did not ensure that employees shut down the equipment and followed a de-energization sequence prior to allowing them to work inside of the Krispy Kreme 270 PX Extruder/Proofer.   (b) On or about August 15, 2024, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of hazardous energy sources were performed prior to employees cleaning the barrel on the Magna Barrel Mixer. Employees were exposed to caught-by hazards associated with the rotating barrel likely to result in serious injuries. The employer did not ensure that employees shut down the equipment and followed a de-energization sequence prior to allowing them to work inside of the Magna Barrel Mixer.   The employer failed to implement energy control application steps as the machines were not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
