29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a) Production area: On or about August 15, 2024, and at times prior, the employer exposed employees to amputation hazards in that they are required to change a circular saw blade on a monthly basis on the Denver Bridge Saw without following specific lock-out procedures. The employer has not established a program consisting of energy control procedures, employee training and periodic inspections.

29 CFR  1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:   a) Facility, Outside North Wall: On or about August 2, 2024, the employer exposed an employee to struck-by hazards, in that a guard was not provided on the Makita Angle Grinder, Model 9564CV, Serial Number 620119A, fitted with a rim diamond blade while cutting a quartz stone slab.
