29 CFR 1910.29(b)(2)(i):29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that mid-rails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface. On or about July 30, 2024, and at times prior thereto, employees conducted cotton gin cleaning operations at the cotton gin stairway by the cotton baling area. The employee fell between the stairway top railing and the stairway tread landing approximately 20 feet below on the cotton gin floor. The employer did not ensure the mid rails were installed at a height midway between the top edge of the guardrail system and the walking-working surface.

29 CFR 1928.57(c)(2)(i): The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets and idlers were not guarded. On or about July 30, 2024, and at times prior thereto, employees conducted cotton gin cleaning operations by the Rescuer 4000 and Moss lint cleaner, the moving belts and pullies were not guarded.
