29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Pizza McKinley: On or about 8/15/2024, employees were exposed to rotating parts and caught-in type hazards when using the emergency stop button to shut down the machine, remove the panel guards, and enter it to clean the rollers and make adjustments, without utilizing proper lockout/tagout procedures. b) 50" FLEXO: On or about 9/16/2024, employees were exposed to rotating parts and caught-in type hazards when using the emergency stop button to shut down the machine, remove guards and make adjustments, without utilizing proper lockout/tagout procedures.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed. a) Jobsite: On or about 8/16/2024, the employer did not ensure that the machine specific energy control procedures for Lockout/Tag-out were inspected annually to include the following machines: Partition; Taper/ Gluer; Stitcher; 37" Flexo; Rotary Die Cut; Corrugater; McKinley/Pizza; Ward 37.5"; 66" Jumbo; Bandsaw; and 50 Flexo". Key Container Corporation was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1441555, citation number 2, item number 1 and was affirmed as a final order on 12/02/2019, with respect to a workplace located at 21 Campbell Street, Pawtucket, RI 02861.
