An employee was observed operating a JLG model #400S, serial # 0300135171 and was not connected by a lanyard to the connection point on the lift. Observed on or about 08/16/24  J & T Brothers, LLC, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 1926.453(b)(2)(v). WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1519366 CITATION NUMBER 2, ITEM NUMBER 1, ISSUED ON 09/01/2021. NOTE: FINAL ORDER DATE 04/07/2022.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
