29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees performing the servicing or maintenance. On or about June 7, 2024, in the HFC-245fa Plant, 5th deck of the HFC-245fa process structure at Honeywell Geismar, Turner Industries Group LLC failed to verify that the Lockout/tagout put in place by Honeywell Interenational Inc. was effective in providing administrative controls, engineering controls, and hardware for isolating, securing, or blocking energy sources. The lack of controls contributed to a Hydrofluoric Acid escape that burned an employee.

29 CFR 1910.119(h)(3)(i): The contract employer did not ensure that each contract employee was trained in the work practices necessary to perform his/her job safely. On or around June 7, 2024, on the fifth floor of Hydrofluoric Acid Unit 245 at Honeywell Geismar, the employer did not ensure that workers followed safe work practices. The workers were exposed to HF spray and burn hazards when the contract employer did not ensure that the Turner employees had knowledge and training of procedures for valve and gasket change per the job package and work permit. Turner Industries Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.119(h)(3)(iv), which was contained in OSHA inspection number #1575813, citation number 1, item number 3 and was affirmed as a final order on September 7, 2023, with respect to a workplace located at 1300 PPG Drive, Westlake LA, 70669.
