29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or about 09/18/2024, throughout establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: changing belts on Slitters. The employer has not developed machine specific energy control procedures, nor has the employer trained or periodically reviewed employees performing covered operations where the employees were exposed to potentially hazardous energy sources and amputation hazards.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) On or about 09/18/2024 throughout establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: changing belts on Slitter #2, employees were not trained to recognize hazardous energy sources in the workplace and the means and methods (procedures) used to control those hazardous energy sources during servicing and/or maintenance of the machines and equipment in the workplace. 29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about 09/18/2024, in the Wheel room; employees were exposed to being struck by flying debris when using the 3/4 H.P. Bench Grinder serial #810250871 to sharpen carbon cutters with a tongue guard adjusted to more than 1/4 inch.    NO ABATEMENT CERTIFICATION REQUIRED
