29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  On August 20, 2024, at the above mentioned address, employees of Cesar Ignacio Medina dba Lara Construction were exposed to eye injury while operating pneumatic nail guns without  eye protection.  No abatement certification or documentation is required for this item.

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a) On August 20, 2024, anchorages being used for attachment of personal fall arrest equipment were not installed so that they would be capable of supporting at least 5,000 pounds per employee attached. 29 CFR  1926.502(d)(17):The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  On or about August 20, 2024, at the above mentioned address, the employees were exposed to fall hazards of approximately of 12.3 feet ground to eave while performing residential roofing work. Employees were connected to the fall protection lanyard at the nylon webbing at their hip.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) On or about August 20, 2024, an extension ladder used by employees to gain access to the working surface did not have its side rails extended 3 feet above the upper landing being accessed.  No abatement certification or documentation is required for this item.
