The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a. On or about August 21, 2024, the employer did not certify that employees had been trained and evaluated on the knowledge and skills need it to operate the Linde lift truck, model H80D-02, serial number E1X353M0039880, and the Caterpillar lift truck, model GP25N, serial number AT35A02484.
