(a) On or about August 22, 2024, at 2137 S. 27th St, Milwaukee, WI 53215, employees installing a roof on a residential home roof with an approximately 6:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 22 feet 6 inches from the roof eve to the ground below on the right (North) side of the home.
