29 CFR 1926.552(a)(1):29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.                                                                   a)  On or about August 22, 2024, the employer provided and allowed employees to use a Garlock Equipment Company's Hand Power Hoist, that was not equipped with approved counterweights.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On August 22, 2024, company employees were performing roofing work to an existing residential structure and utilizing an aluminum ladder to access the work area that did not extend three feet past the edge of the roof being accessed.  No abatement certification or documentation is required for this item.

New Age Roofs, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a)(1) which was contained in OSHA inspection number 1520064, citation number 1, item number 1, and was affirmed as a final order on June 21, 2021, with respect to a workplace located at 2743 72nd Ct, Elmwood Park, IL 60707.  No abatement certification or documentation is required for this item.

The employees were exposed to falls between approximately over three stories above the ground.  New Age Roofs, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1520064, citation number 1, item number 2, and was affirmed as a final order on June 21, 2021, with respect to a workplace located at 2743 72nd Ct, Elmwood Park, IL 60707.
