29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 23, 2025, the employer did not ensure all employees working above heights greater than six feet or more above a lower level were protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  New Age Roofs, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1770675, citation number 2, item number 2 and was affirmed as a final order on 10/15/2024, with respect to a workplace located at 915 Woodward Ave Deerfield, IL 60015.  New Age Roofs, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1520064, citation number 1, item number 2, and was affirmed as a final order on 6/21/2021, with respect to a workplace located at 2743 72nd Ct, Elmwood Park, IL 60707.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about April 23, 2025, the employer did not prepare a written training certification records for employees to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards for employees working at heights greater than 6 feet.
