The employer did not ensure that employees utilized a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 3080 E. 142nd Pl, Thornton CO.: On or about August 15, 2024, and at times prior, employees were exposed to falls of approximately 20 feet to the ground below while accessing/egressing upper levels of the existing residential home. The ladders were not secured at their top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladders.
