At an establishment located at 1695 Mills Street in Green Bay, Wisconsin; the employer did not develop, document, and utilize a procedure for the control of potentially hazardous energy when maintenance employees were engaged in servicing and rebuilding Tri-Clover clean-in-place seat valves, exposing employees to amputation hazards.

29 CFR 1910.147(d)(4)(i): Lockout or tagout device application. Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. At an establishment located at 1695 Mills Street in Green Bay, Wisconsin; maintenance employees were exposed to machine hazards associated with moving parts when they engaged in servicing and rebuilding Tri-Clover clean-in-place seat valves. The employer did not ensure that authorized employees working on the valves affixed a lockout device to each energy isolating device.
