29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) On or about August 29, 2024, at 1227 Williams Ave, South Milwaukee, WI 53172, employees conducting roofing operations on an existing residential home were wearing full body harnesses but had their anchor point rope attached at the waist of their harnesses and not in the center of their backs. This exposed the employees to a fall hazard of approximately 8 feet 9 inches on the front (north) side of the home.
