29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Kups, Inc. Loading Dock. On or about September 5, 2024 a duplex outlet located between the overhead doors of the loading dock was equipped with a damaged faceplate, exposing employees to live conductors inside of the outlet box.
