29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. Location -Platform on roof of Fabric Filter #1 Building, #1 Deflation Fan. On or about 9/3/2024, the employer did not train employees on the lockout/tagout procedures for the #1 deflation fan so that the hazardous energy could be controlled prior to performing preventative maintenance tasks. The hazardous energy to be controlled for the #1 deflation fan includes, but is not limited to, electrical, moving dampers (air), and rotating fan blades.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). Location - Platform on roof of Fabric Filter #1 Building, #1 Deflation Fan. On or about 9/3/2024, the baghouse dampers connected to the #1 deflation fan were not physically locked out and tagged out with energy isolating devices prior to performing preventive maintenance on the #1 deflation fan. Stored energy within the dampers was not relieved, disconnected, restrained, and otherwise rendered safe.
