29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Location - Superheater boiler hopper in boiler #2 - "Elevation 62".  On or about 7/1/2025, NOMEX suits and other such personal protective equipment (PPE) to protect employees from burn hazards were not utilized when cleaning out hot ash from the superheater boiler hopper.
