Location: 42 Canal Street Shelton, CT 06484 On or about September 12, 2024, the employer did not require proper/specific energy control procedure shut down and/or alternate protection for an employee clearing a machine jam of the Lyle #7 trim press resulting in the partial amputation of an index finger.

29 CFR1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source. Location: 42 Canal Street Shelton, CT 06484 On or about September 12, 2024, the employer did not ensure the main pneumatic feed valve to the Lyle #7 trim press was operated in such a manner as to isolate machinery components from stored energy, while an employee cleared a jam near ski/roller components in the discharge area.
