29 CFR 1910.178(n)(12): Elevators shall be approached slowly and then entered squarely after the elevator car is properly leveled. Once on the elevator, the controls shall be neutralized, power shut off, and the brakes set. 34 CANAL/34 CANAL ELEVATOR: On/or about July 24, 2025, and prior to, the employer did not ensure the forklift operator who was operating the Yale FL6 powered industrial truck approached the elevator slowly and that the elevator car was leveled with the ground resulting in an incident where the forklift and employee fell into the elevator shaft.
