29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2)  On or about August 17, 2024, at the facility located at 1250 South Hickory Street, Fond du Lac, Wisconsin; the employer did not ensure that the stairs used by employees to access the old Supervisor's Office in the old shop area were equipped with handrails and stair rails to prevent falls.  The upper section of the stairs was not equipped with a handrail.

29 CFR 1910.179(j)(3): Periodic, complete, inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated in section 1910.179(j)(4). These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the items identified in sections 1910.179(j)(3)(i)-1910.179(j)(3)(x). Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard. 29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions.  [55 FR 13709, Apr. 11, 1990; 61 FR 9227, March 7, 1996; 70 FR 53929, Sept. 13, 2005]  On or about May 9, 2022, at the facility located at 1250 Hickory Street, Fond du Lac, Wisconsin; an employee was exposed to potential contact with electrical current on the Lincoln Electric Co. Power MIG 350MP (Machine No.: 11147, Serial No.: K2403-1) welder.  The ground wire had damaged insulation and was not replaced.
