29 CFR 1926.1053(b)(1):Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top. a) On or about September 25, 2024, at the jobsite: employees were exposed to an 11'- 4" fall hazard while accessing the 4/12 pitched roof, in that the ladder was extended approximately 1 foot past the roof landing.
