29 CFR 1910.119(m)(4): An incident investigation report was not prepared at the conclusion of the investigation to include at a minimum: a description of the incident; the factors that contributed to the incident; and any recommendations resulting from the investigation: Facility: The written Incident Investigation report for a release of anhydrous ammonia that occurred on November 21, 2023 did not include a description of the incident, factors that contributed to the incident and any recommendations resulting from the investigation. The documentation provided by the employer consisted of a checklist with handwritten notes for action items and an email attachment that detailed issues that arose during the response to the incident. The employer did not have a full accounting of the incident or any documentation to show that any of the issues mentioned in the report had been addressed.
