29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. 47 Haskell Road - On or about September 26, 2024, an employee was hospitalized, and the event was not reported to OSHA until September 30, 2024. Sam's Club was previously cited for a violation of this occupational safety and health standard 1904.39(a)(2), which was contained in OSHA inspection number 1633535, citation number 1, item number 1 and was affirmed as a final order on 12-15-2022, with respect to a workplace located at 4831 13th Avenue South Fargo, ND 58103.
