29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   a) On or about August 28, 2024, the employer did not ensure an employee operating the KOCH hydraulic embosser press in the embossing area was protected from the caught-in hazard between the press and pattern die by one or more methods of machine guarding.  The press lacked guarding that kept employees from reaching the point of operation. Employees were assigned to operate the press while portions of existing guarding including, but not limited to bump guards were not functional.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On April 28, 2024, the employer did not report the in-patient hospitalization of an employee within 24 hours of an employer representative learning of an employee being hospitalized.
