29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about October 1, 2024, at the Grieve Oven #1, the employer had not developed a lockout/tagout procedure for the control of potentially hazardous energy, such as but not limited to electrical, thermal, pneumatic, and natural gas, for employees who performed servicing and maintenance. Thereby exposing employee to electrical shock, caught in hazards, and explosion hazards.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) On or about October 1, 2024, the employer had not ensured that employees tasked with routinely handling hazardous materials such as but not limited to, Barsol A-4794 and Bondrite S-OT 310B, were provided with an eye wash station suitable for flushing of the eyes for 15 continuous minutes.

29 CFR  1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  (a) On or about October 1, 2024, the employer had not ensured that the emergency exit located on the north side of the facility was free of obstructions.
